- The CMS-855I application is used for individual provider enrollment in Medicare and for reassignment of benefits to a group. It can be used by both physicians and non-physicians.
- The CMS-855B application is used for Clinics, Group Practices, and Certain Other Suppliers like independent diagnostic testing facilities.
- The CMS-855S application is used for DMEPOS suppliers.
- Providers may also need to submit the CMS-460 form to elect for participation in Medicare, without this form providers may be enrolled as nonparticipating providers. Nonparticipating providers will receive less reimbursement from Medicare, though they are also entitled to pursue more reimbursement directly from patients—up to 115% of the Medicare rate.
- The CMS-588 form is used to have your Medicare payments deposited directly into your bank account. Medicare does not issue paper checks; reimbursement is only dispensed via EFT. Individual providers do not need to submit a CMS-588 when reassigning their Medicare benefits to a group practice.
